Hi, my name is Emma.

I am an independent support worker and End of Life Consultant situated on the picturesque Fleurieu Peninsula in South Australia. The journey of my support worker role commenced nearly five years ago while residing in the scenic Blue Mountains. I hold a Diploma in Outdoor Recreation, a Cert IV in Massage Therapy, a Diploma in Community Services, and a Cert IV in Health Administration. Most recently, I broadened my expertise through training in End of Life Doula and Consulting practices.

I offer assistance to individuals of diverse spiritual and religious affiliations, BIPOC, various races, and gender identities. My goal is to create a personalised experience that aligns with the unique needs and preferences of each individual.

I possess experience collaborating with a broad spectrum of clients, which includes, but is not restricted to:

  • Elderly in Nursing homes
  • Individuals approaching the end of life or individuals in the final stages of their lives.
  • Assisting individuals who are actively engaged in life but seek guidance in organising their End of Life Planning for potential life changes.
  • Family of loved ones who are nearing End Of Life and need support with planning.

  • Acquired Brain Injuries
  • Recovering from injury i.e fall
  • Mental health and emotional support
  • Individuals across the lifespan, encompassing children, teenagers, and adults, who are diagnosed with Autism and/or ADHD.
  • Individuals in search of support for community engagement, acquiring social skills, confidence, and managing daily tasks.

In my leisure time, I find immense joy in rock climbing, a passion that has led me to explore various destinations across Australia, Europe, South America and most recently, South Africa. Traveling for this obsession has not only allowed me to experience different cultures but has also been a profound journey of self-discovery. With a background in Outdoor Recreation and Tourism, I’ve served as an outdoor adventure guide for several years. The outdoors holds a special place in my heart, and there’s something incredibly rejuvenating about the fresh air that keeps drawing me back.

I am a friendly, approachable, and empathetic individual with deep respect for others. Drawing from personal experience, I have a profound understanding of mental health. My interests include cooking, connecting with animals, and assisting people with domestic and organisational tasks. I genuinely hope to be a source of support for you.

My NDIS participant rates are determined in accordance with the NDIS support catalogue for the year 2023/2024.

My support work rates for clients not covered under the NDIS are as follows:

Weekday $65/hr

Weekday afternoon (after 6pm) $70/hr

Saturday $80/hr

Sunday $95/hr

Public holidays $110/hr

I will invoice you, your Plan Manager or Support Coordinator directly.

If you are not an NDIS participant, and these rates are not within your budget, kindly get in touch with me. I am open to discussing potential adjustments to better accommodate your needs.

Advanced Care Planning will help guide your team when you are no longer able to speak for yourself and can significantly contribute to easing the grieving process in several ways:

Clarifies End-of-Life Preferences:

  • Advanced care planning allows individuals to express their preferences regarding end-of-life care, including medical treatments, interventions, and the level of life-sustaining measures they wish to receive. This clarity can provide comfort to both the individual and their loved ones during difficult decision-making moments.

Reduces Family Conflict:

  • When a person’s end-of-life wishes are clearly documented through advanced care planning, it can reduce the potential for family conflicts. Knowing and respecting the individual’s preferences can minimise disagreements among family members about medical decisions, ensuring that choices align with the person’s expressed desires.

Facilitates Open Communication:

  • Engaging in advanced care planning promotes open and honest communication between individuals and their loved ones. This communication can enhance understanding, alleviate anxiety, and foster a supportive environment. It allows for important discussions about values, beliefs, and expectations surrounding death and dying.

Provides Emotional Support:

  • Knowing that one’s preferences for care are documented and will be honoured can provide emotional support to individuals facing the end of life. This sense of control and agency over their medical care can ease anxiety and contribute to a more peaceful transition.

Supports the Grieving Process for Loved Ones:

It provides a roadmap for making decisions when the individual is no longer able to communicate their wishes. This can alleviate the burden on loved ones and allow them to focus on providing emotional support to the person in their final moments.

  • Advanced care planning encourages timely decision-making, ensuring that important discussions about end-of-life care occur before a crisis. This proactive approach allows individuals and their families to address difficult topics in a calmer and more thoughtful manner, reducing stress during critical moments.

  • By clearly expressing one’s preferences for end-of-life care, advanced care planning helps ensure that the individual’s wishes are respected. This can lead to a more dignified and personalised end-of-life experience, aligning with the person’s values and beliefs.

  • First part of session: Take the time to share elements of your life, interests, beliefs, and experiences at a level that feels comfortable for you. This helps facilitate the subsequent planning steps.
  • middle of session: Create an in depth plan together, map out and document your preferences.
  • middle to end or subsequent sessions: Option to complete the Advance Care Directive for your State/Territory, and then follow up to confirm all necessary signatures are obtained, making copies for future reference.
  • Organise a personal folder to safely store your documents and position fridge magnet which will state where they are located. (Folder and magnet included for in person consults)
  • Consultations offered in home, via phone/Zoom or at a safe and confidential workspace.

I have a 16 year old daughter who suffers with ASD and ADHD. Emma takes her out for bush walking or rock climbing with a hot choccy to finish up their session. My daughter loves her outings with Emma and she looks forward to her coming every other week. Emma is amazing she will aim to fit into your schedule. Emma is always on time and has a wonderfully calm personality. My daughters exact words were “Emma is so nice and funny!”.

Notes and client details collected will be retained and stored safely for seven years in accordance with the terms of Emma Cooper’s Professional and Liability Insurance.

These sessions do not serve as a substitute for treating medical conditions, mental health concerns or discontinuing prescribed treatments from licensed medical professionals.

Your choices and decisions remain solely yours.

It is strongly advised to consult your GP and certified health practitioner for any health or mental health concerns.

Emma Cooper reserves the right to cancel a session immediately if she perceives any threat or feels unsafe during the appointment, rendering any refunds null.

Respectful behaviour is expected and greatly appreciated at all times.

PAYMENT AND CANCELLATION POLICY

Payment must be submitted at least 72 hours (3 days) before the scheduled appointment commencement or, for short-notice appointments, within 24 hours upon receiving the invoice, depending on the situation.

Failure to do so will result in appointment cancellation, and I will make every effort to contact you before taking this step.

If financial difficulties arise, please reach out to discuss before scheduling an appointment.

For recurring appointments, advanced payments can be arranged with weekly, fortnightly, or monthly instalments.

Cancellations will be considered under special circumstances, and refunds are evaluated case by case, subject to Emma Cooper’s discretion.

No refunds are provided for missed appointments. In such cases, re-booking requires payment to confirm a new appointment.

Late Arrival Policy:

Punctuality is essential. If you are running late, please make contact. Any lost time is not the responsibility of the service provider.

Change of Appointment Time Policy:

Once payment is made, date and time changes must be requested at least 24 hours before the original appointment and will only be considered under special circumstances.

If you need to cancel your appointment:

In the unlikely event of appointment cancellation by the service provider, you will be notified as early as possible. A full refund for that appointment will be issued if an alternative date cannot be arranged.

These terms and conditions constitute the agreement between you (the Client) and Emma Cooper.
